Full-time residents must show:

• Florida Driver’s License or Florida State ID

Seasonal residents without a Florida ID must provide two of the following, addressed to a Florida residence:

• Residential lease, mortgage, or deed
• Utility hookup or work order (within 60 days)
• Utility bill (electric, water, gas, within 2 months)
• Bank/financial institution mail (within 2 months)
• Government agency mail (within 2 months).

Florida recognizes numerous qualifying conditions including, but not limited to:

Physicians may also approve comparable debilitating medical conditions.

You’ll meet with Michelle Ayazo, MD a board certified state-licensed medical provider who evaluates your condition and, if appropriate, enters your certification and treatment orders into the Florida Medical Marijuana Use Registry.

Your certification is valid for 210 days (7 months).

A follow-up appointment is required to continue treatment after that period.
